These are the Privacy Statement referred to in the Working Together Coaching & Therapy Agreement between us
What data do we keep and why do we need it?
Name and age – this is basic information that helps us get to know you.
Address, email address, phone number – we use this as a way of contacting you regarding your sessions. we will mainly use the method you first contacted us on but if we cannot reach you, we will try a different method.
Next of kin/medical professional’s details – If we were worried that you were at risk then we may need to contact your next of kin or medical professional, if we can. We will let you know when/if we are going to do this.
Session notes – We keep brief notes of our session(s), these are kept securely in a locked cabinet. All emails are saved on a password protected laptop.
Will we share your data and if we do, who will we share it with and for what purpose?
It is very unlikely that we will share your data. We will not sell it on or use it for unethical reasons. We may have to share it if our notes are ordered by court, if you or anyone you tell us about is at harm or risk of harm, we may have to pass this information on. We may also discuss your case during supervision, but we only use your first name.
How will we store your data?
Notes are mainly stored as hard copies in a locked filing cabinet. Any emails are saved on a password protected laptop. Your phone number will be kept in a mobile phone with your first name and last initial for the purpose of sharing audio files. Only we will access your information. Your name, email, phone number, submitted forms and signed T&C’s data will also be retained in the website admin back office and this complies with the GDPR rules.
How long will we store your data for and how will we dispose of it?
We will keep your details and session notes for the time required by our insurer (currently 5 years).
After this time, we will destroy any document with your personal information and delete your phone number out of the mobile phone.
Cookies
If you choose to contact us on our site you are in effect opt-in to saving your name, email address, contact phone number and website in cookies. These are for your convenience so that you do not have to fill in your details again if you request another zoom call. These cookies will last for one year.
If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.
When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select "Remember Me", your login will persist for two weeks. If you log out of your account, the login cookies will be removed.
What rights you have over your data
If you have an account on this site, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
